The below details everything you need to know about the Annual Provider Patient Safety Module – due by noon, Monday, December 7, 2015.

You may also download a PDF fact sheet to guide you through the process. Click here to access.

Completion of the Annual Provider Patient Safety Module is one of the ways we demonstrate our commitment to and creation of a safe environment here at Emory Healthcare. All providers at EHC must complete the 2015 module by noon, Monday, December 7, 2015, to retain their privileges. Thank you for keeping Emory Healthcare a safe place for our patients and their families.

See below for details on requirements, how to know you’ve completed the module and help contacts.

Four requirements – Deadline is noon, Monday, December 7, 2015

1. Influenza vaccination requirement:

  • Get vaccinated. You can get vaccinated for free at an EHC flu vaccination event or Employee Health (click here for locations). For a schedule of flu vaccination events, log on to the Virtual Desktop, visit the home page of the intranet (www.ourehc.org) and click the influenza button, or contact Employee Health. The vaccine is mandatory for all providers, except those with documented medical contraindications or a religious exemption form signed by their religious leader or hospital chaplain. Egg-free vaccines are available at Employee Health locations; an egg allergy cannot be applied to the medical contraindications exemption.

If you are an EHC- or EU-employed provider and you are planning on getting vaccinated at Employee Health, you must complete the online consent form (pre-registration) prior to getting vaccinated by Employee Health. To pre-register, log on to e-Vantage, then click Self Service and Influenza Vaccine Registration to complete the form. (e-Vantage is accessible via the right-hand navigation on the intranet, www.ourehc.org. Log on with your network ID and password.)

Non-EHC-employed providers (whether or not you get vaccinated by Employee Health) or those EHC- or EU-employed providers being vaccinated outside of Employee Health do not need to complete online pre-registration.

  • Document having received the vaccination in the Annual Provider Patient Safety Module. To log in, click on the “Click here to access Provider Patient Safety Module” box at the bottom of this page.

  • Submit documentation if you receive the vaccination outside of Employee Health to Cindy Hall at cynthia.hall@emoryhealthcare.org or fax to 404-686-1527. Or, submit a completed medical or religious exemption form (form available on the Annual Provider Patient Safety Module) to Cindy Hall. The religious exemption form must be signed by your religious leader or hospital chaplain. Remember: You must apply for exemption each year.

2. TB test requirement:

  • Get the TB test (tests performed within the past 12 months are permitted), unless previously tuberculin skin test (TST) positive. You can obtain a TB test at Employee Health (click here for locations) at no cost. Private practice providers can go to any of the Employee Health offices to have the TB test placed and read. If not done at Employee Health, TST should be placed and read by an appropriately qualified individual.

  • Record your TB test by entering your TST and/or T-spot (if you have a history of a past positive TST or T-spot) date and results in the TB section in the Annual Provider Patient Safety Module.

  • NEW THIS YEAR If you do not have your TB test placed and read at Emory Healthcare Employee Health, you must submit documentation of your TST and/or T-spot results to Cindy Hall in Employee Health at cynthia.hall@emoryhealthcare.org or fax to 404-686-1527.

TST documentation must include the site of the TST placement (L or R), date placed, date read, expiration date of PPD solution, lot number of PPD solution, reader’s name and mm size of reading. T-spot documentation can be simply a copy of the lab results. We are changing our TB requirement to include this documentation based on feedback from DHR (Department of Human Resources) surveyors.
