Patient/Visitor Guide

Welcome to the Patient/Visitor Guide for Emory Dermatology.  We hope the following information is useful to your visits to see us.


Patient Registration


Before your first appointment with a physician from the Dermatology Section, you may want to complete the medical history forms below. We also request that you obtain medical records from your other health care providers and bring them with you if, possible.

These steps will help us to expedite your appointment and ensure that we have the appropriate information needed to provide the best advice possible during your initial consultation.

You may print the form, fill in all the information, and return it by mail to the office of the physician with whom you have an appointment, or you may bring it with you to your first appointment. We cannot accept the form electronically due to patient confidentiality.

First Visit


You may make an appointment with a physician from the Dermatology Section of The Emory Clinic by contacting us directly or through a referral from your primary care physician.  Because of great demand for our speciality services and limited numbers of physicians with the unique expertise required to provide these services, we may not be able to immediately schedule an appointment.

In order to facilitate the appointment process, it would be helpful for you to send any reference notes from your physician and test results, such as previous skin biopsy and laboratory results, if applicable. You should also bring this information to your initial evaluation. In addition, a patient registration form may be found by clicking here or going to the appropriate section of the site. The form should be sent to us in advance of the appointment.

You should also bring medical history information, a list of your current medications and your insurance card. Many dermatologic conditions are covered by most insurance carriers and in most cases, you can be seen and the visit is covered without getting a referral. However, you should check with your insurance provider about your particular options before making an appointment. Patients whose insurance requires a referral prior to being seen will not be evaluated without a referral.

Location (Campus Map)


The main offices of the Emory Department of Dermatology are located at The Emory Clinic, Department of Dermatology, Building A, at 1365 Clifton Rd. in Atlanta, Georgia, 30322.

Please plan for traffic congestion in the immediate vicinity around the Emory campus, particularly during rush hours.

Parking: A parking deck is located behind Building B, just across the street from Building A. The cost for parking is $5. There is also Valet parking for $7 in front of Building B.

The Dermatology Section of The Emory Clinic and the Department of Dermatology at Emory is dedicated to helping our patients cope with physical and psychological distress that may be a result of many skin conditions.
