Referral Process
Upon receipt of a referral, the referring provider is faxed a confirmation letter which includes notification of the patient’s education class appointment date. All patients referred for a Kidney, Kidney/Pancreas, or Pancreas Transplant evaluation will be scheduled for a kidney/pancreas education class. A letter will be mailed to the patient informing them of the date they are scheduled for class. All patients must attend the education class to continue in the transplant evaluation process.
If a patient does not attend a scheduled education class the "No Show Policy" will be implemented. The Emory medical secretary senior will attempt to contact the patient three times using all phone numbers provided including the dialysis center and referring nephrologist. If no contact is made after three attempts, a letter will be sent to the patient and referring nephrologist indicating that we will not further pursue our efforts to evaluate the patient for transplant.
Patients who attend the educational class will decide one of two possible outcomes:
- Move forward with their evaluation by signing the Kidney/Pancreas Transplant Program’s Letter of Intent
- The patient determines they are not ready to pursue transplantation at this time and they check the box indicating this and sign the Letter of Intent. A letter is sent to the referring physician and the patient stating that we will not proceed with the transplant evaluation.
