Medical Records and Release of Information

Attention patients and patient representatives: The walk-up window for requesting copies of medical records at our Emory University Hospital Midtown location is closed indefinitely. Please make your request by mail, fax, email, or electronically per the instructions below for this facility.

If you are seeking a copy of your covid-19 vaccine record, please choose "Emory Clinic" as the facility in the online tool. If you are an Emory Healthcare employee who received your vaccine through Emory Occupational Health, please use the HOME portal to print a copy of your vaccine record under the “My Medical Information” tab.

For after-hours, weekend, and holiday urgent continuation of care requests for records, please call the following facilities/numbers:

Your health record includes your medical history such as allergies, medicines, and test results, as well as, health insurance information and how to contact you. It is the duty of Emory Healthcare to ensure that your record remains confidential and is maintained and released in accordance with applicable laws. To request access to your patient health information, please select an option below.

Electronic Request for Records

Emory Healthcare now offers an Online Patient Record Request tool for the following individuals to request patient records:

  • Patients requesting their own records
  • Parents of minor patients requesting records
  • Caregivers acting on behalf of a patient (i.e. Power of Attorney)

The Online tool verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

Please note, Chrome, Safari and Firefox are the recommended browsers for this application. If you encounter any issues with your electronic request, please call our Release of Information Call Center at 404-712-4111.

Our paperless request process does not require a login but uses email verification for enhanced security. Faxing, scanning, or printing will not save you time. Please use a recent version of Chrome, Firefox or Safari.

Paper Request for Records

To have a copy of your medical records sent to another medical facility or to obtain a copy for personal reasons*, please complete the following steps:

Step 1: Download and complete the Emory Healthcare Authorization for the Release of Protected Health Information forms

Step 2: Fax, mail, or email the form to the Health Information Management (HIM) Department of the Emory Healthcare facility (see list below) indicated on the form
