We changed our referral process to reduce wait times for an initial evaluation and improve timely care. A referring provider is now requested to start the patient’s journey, assist with ordering basic labs and brain imaging, and collaborate with our team for ongoing patient care. In some cases, a data-only review (like an eConsult) may be performed prior to or instead of a face-to-face visit with a cognitive provider. Please share the instructions below with your referring provider.

Instructions for your referring provider on Emory EPIC:

• Search for the ‘Cognitive Neurology Clinic Referral Order Panel’ in Orders.
• Follow the instructions at the top of the order panel.

• You will need to complete brain imaging and labs if ordered by your referring provider.
• Our team will reach out to schedule a memory testing appointment and provide additional instructions.
• Sign up for MyChart. MyChart is an important way to communicate with your care team, review results and notes, and even upload brain images for your provider to review.
• Complete the cognitive clinic questionnaire in MyChart during the week before your memory testing appointment.

Instructions for your referring provider NOT on Emory EPIC:

• The referring provider should sign up for Emory Healthcare Link.
• Brain imaging (MRI brain or CT brain if MRI contraindicated; within last two years) and labs (B12, TSH, CBC, and CMP; within last year) are requested in Emory EPIC for the referral process to the Emory Cognitive Neurology Clinic.
• Through Emory Healthcare Link, place orders for:
  • First, search for the Emory Cognitive Clinic Referral order panel and follow the instructions. If this order panel is not available, use the orders below.
  • ‘Ambulatory Neurology referral’ and select ‘cognitive’ subspecialty
  • ‘MRI brain without’ and select ‘dementia’ protocol. Use ‘Head CT without’ if MRI is contraindicated. Provide the patient with the radiology scheduling number (404-778-9729).
  • ‘CBC’, ‘CMP’, ‘TSH’, and ‘B12 level’
• Existing brain images must be shared with Emory, and existing labs and clinic notes should be faxed (404-712-2787) to help expedite the care of your patient.
• As the referring provider, please place this phrase in the patient clinic note: “The patient consented to a referral for cognitive testing, an Emory Cognitive Clinic eConsult, and provider evaluation.”

• Complete brain imaging and labs if ordered by your referring provider.
• Our team will reach out to schedule a memory testing appointment and provide any additional instructions.
• Sign up for MyChart. MyChart is an important way to communicate with your care team, review results and notes, and even upload brain images for your provider to review.
• Complete the cognitive clinic questionnaire in MyChart during the week before your memory testing appointment.
