The Self-Management Training Program is recognized by the American Diabetes Association. We are honored that you and your doctor have chosen us to help you learn more about how to successfully manage your diabetes. The American Diabetes Association has developed a three-session program that focuses on seven key areas.
Sessions
1. “One-on-one” Evaluation – First, you will meet with a certified diabetes educator for one hour. During this meeting, we will ask you how you currently manage your Diabetes and provide you tips to help you.
2. Group Class – The next step is to attend a group class at one of the many Emory Healthcare locations across Metro Atlanta.
The group classes include education and discussions about:
- Healthy Eating
- Problem Solving
- Reducing Health Risks
- Healthy Coping
- Taking Medication
- Being Active
- Monitoring Blood Sugar
The group setting is a very supportive environment where lots of questions are answered and you and your classmates have a chance to participate in the discussion.
3. Follow-Up Group Class – You will be automatically scheduled for a one-hour follow-up class, which will take place two months after you finish the group class. Please bring a copy of your blood glucose logs and A1C to this class, if available.
Attending all three sessions is important for the best results in improving your blood sugar levels. Our goal is for you to live your life to its fullest and healthiest!
