You can call our office at 267-534-0452 to schedule an appointment. Please arrive for your appointment about 15 minutes early to allow ample time to complete your paperwork and confirm your demographic and insurance information. We do not accept checks; however, we do accept all major credit cards, debit cards and cash payments.

Bring a complete list of all of your medications. If this is a return visit, let us know if there have been any changes in medication or additional medical problems since your last appointment. We need to know this so that we can update your health history and chart. The more we know about you, the better we can serve your health needs.

We do same-day appointments. We are not accepting walk-ins at this time. We prefer to be able to schedule you so that you don't have to wait a long period of time to be served. If you need to see us, give us a call and we can fit you in for the day.

Please let us know 24 hours in advance if you will be unable to keep your appointment. We do apply a $25 charge for missed appointments that are not canceled before the 24-hour window.

If you call for a prescription refill between our business hours of 8 a.m. and 5 p.m., we do our very best to handle your request on that day. However, we can assure that your request will be taken care the following business day. We do charge $20 to redo a lost prescription.

Our on-call physician will not be able to call in your refill request over the weekend, or during the overnight. Please try and make your request during our business hours.

We will not authorize a prescription if we feel that you have not been seen by your physician in a reasonable time span. We also will decline to authorize a prescription if we are concerned that a patient is not complying with the treatment plan, or is abusing the medications.
