- Obtain any referrals required by your insurance several days prior to your appointment date.
- Bring a copy of your insurance card.
- Bring or write down the medications you are currently taking and the dosage.
- Bring your treatment calendars (bleed logs) and any other health information that you want to share.
- Write down your health goals and questions you may have for the bleeding disorders team.
