Phase 1 Cardiac Rehabilitation
Phase 1 is an inpatient program. It is for people who have had a heart attack or another cardiac issue.
This phase often includes physical therapy and education. The goal is to strengthen your heart. It also helps reduce your risk of another heart condition in the future.
Phase 2 Cardiac Rehabilitation
Phase 2 is an outpatient service for people who have experienced conditions including:
- Controlled chest pain
- Heart attack
- Heart procedures (balloon angioplasty, stents, coronary artery bypass surgery and heart transplant)
- Heart valve disease (repair or replacement)
- Stable, chronic heart failure with left ventricular ejection fraction ≤ 35%
- Heart or heart and lung transplant
You complete all exercises with electrocardiogram (ECG) monitoring and telemetry. These tools give you instant feedback on how hard your heart is working.
A team of nurses, exercise specialists, cardiologists and registered dietitians supervise your plan. They help you meet goals, manage your health and reduce your risk of recurrence.
Phase 3 Cardiac Rehabilitation
This outpatient program is available after you complete Phase 2. You can also participate if you want to prevent heart disease.
Our trained rehab staff develops a personalized exercise plan with you and oversees your sessions. During this phase, you can take group exercise classes.
Your case manager offers support for making significant healthy lifestyle changes, which can improve your heart health.
